Communities of Practice

REGISTRATION & PARTICIPATION

This convening will bring together key project stakeholders for shared learning, collaboration, and peer exchange focused on strengthening inclusive service delivery and cross-system coordination. Participation in the Community of Practice is required for project operators and is designed to support implementation, problem-solving, and systems-level improvement. Project operators are responsible for coordinating and covering all travel and lodging for their staff. We encourage project operators to plan accordingly and look forward to welcoming teams from across the state.

These materials are designed to support shared learning, inform discussions, and ensure that sessions reflect current project realities across sites. To support meaningful dialogue and full participation during the convening, we kindly ask attendees to complete all pre-event materials by the indicated deadlines.
